Astros outfielder Justin Maxwell left Tuesday night's game against the Cardinals early with a mild concussion.

Maxwell was taken out of the game in the fourth inning after trying to make a catch on a three-run triple by Matt Carpenter.

Maxwell dove, didn't catch the ball, went down hard, and stayed on the ground after the play. TV replay showed that Maxwell banged his head after hitting the field.

The concussion comes a week after Maxwell rejoined the Astros' lineup. He had been out since a pitch broke his hand on April 23. His rehab assignment began on June 4. Since returning, Maxwell has five hits in 21 at-bats, including an 0-for-1 performance before being taken out of the game on Tuesday. J.D. Martinez took over in right field.
